FORM 1095-C – AVAILABLE UPON REQUEST

Sakuma Bros. Farms & Processing is an Applicable Large Employer under federal law. In accordance with the Paperwork Burden Reduction Act and IRS Notice 2025-15, we are not automatically mailing Form 1095-C to all employees.

Form 1095-C reports information about the health coverage offered to you during the calendar year.

You May Request a Copy

If you would like to receive a copy of your Form 1095-C, you may request one at no cost.

How to Request Your Form 1095-C

Please include your full name, current mailing address, and the last four digits of your Social Security number when making your request.

When You Will Receive It

If you request a Form 1095-C, we will provide it to you no later than 30 days after your request, or by January 31 of the following year, whichever is later. This notice will remain available on our website through October 15 of the applicable reporting year.
